Travel claim forms

Making a claim: You can make a claim in two ways:

  1. Download the appropriate claim form by clicking on the "Irish or UK claim forms" menu on the left. You will require Adobe Reader to download the forms.
  2. If you wish to receive a form by post simply email us  using "Request Forms "text in subject heading.   Please include all details of the required information including your personal details such as name, address and click on the send button. Your claim form will be forwarded to you by next day post.
